Millions of litres of crude oil have spilled into the Gulf of Mexico since the catastrophic blow-out of the oil rig Deepwater Horizon in April. How could such a disaster happen and who is to blame?

60 Minutes reporter Scott Pelley talks to the people who should know, including the rig's chief electronics technician Mike Williams, who was one of the last to escape the inferno, jumping 30 metres into an oily black sea.
