Backs Paul Jordaan and Patrick Lambie failed fitness tests to allow Meyer Bosman into the midfield in the Sharks' only change for their Super Rugby semifinal against the Stormers.
Bosman joins Tim Whitehead in the centres for the match at Cape Town.
Jordaan scored a try to help knock out defending champion Queensland 30-17 in Brisbane last weekend, but a hamstring injury has cost him any chance this week. Lambie, the Springboks first-five and Sharks leading point-scorer this year, hasn't played in a month because of an ankle complaint.
The return of prop Brok Harris and Springboks lock Eben Etzebeth are the only changes in the Stormers.
Harris, recovered from a broken nose, will start with tighthead backup Frans Malherbe likely to finish Saturday's match at Newlands, while Etzebeth was over an ankle injury, bumping De Kock Steenkamp to the reserves.
They're the only changes to the side which beat Melbourne 26-21 nearly two weeks ago to top the round-robin.
Duane Vermeulen lost his race on Wednesday to be fully fit from a knee injury two months ago, with hooker Deon Fourie staying at No. 8. Canada international Jebb Sinclair was ineligible for the playoffs.
